WHAT DO I NEED TO BRING?
*The following information applies to everyone in the household. Failure to bring in ALL required information can result in your application being denied.*
Family Composition
Social Security Cards and Birth Certificates
All Sources of Income (We must have your information in writing; phone calls are not accepted for verification; all documents must be current and not older than 30 days.)
• Social Security Benefits Letter (Must be the letter from Social Security Administration. You will have to call the 1-800-772-1213 and request proof of income letter. If you receive Social Security Benefits on an EBT card, like but not limited to a Direct Express Card, and don’t write checks, we must still have 1 month of account statement. Call the number on the back of the card that you receive the money on and request the most current statement.)
• Supplemental Social Security – SSI Letter (Must be the letter for the Social Security Administration. If you receive SSI Benefits on an EBT card, and you don’t write checks, we must still have 6 months of account statement. Call the number on the back of the card that you receive the money on and request them.)
• TANF
• Veteran’s Administration Benefits (VA)
• Military Pay
• Annuities and Interest Income
• Pensions and Retirement Income
• Workman’s Compensation Payments / Short Term Disability Payments
• Alimony
• Child Support ( A Notarized Letter or Payment Statement from DHR Online System)
• Family Donations (A Notarized Letter written by the person that gives you money or supports you.)
• Check Stubs from Employer (4-6 consecutive pay check stubs)
• Unemployment Statement from the Department of Labor with WBA amount
• Income from Real Estate or Personal Property (If you own a house or land or have other assets that you have an insurance policy on.)
• Life Insurance Policies – Term and Whole Life Policies
• Checking and Savings Account Statements (6 Statements are needed or a letter from the bank with a 6 month average balance for Checking and the current balance for Savings; If accounts earn interest this must be reported.
Utility Bills
• Current Power Bill, Gas Bill, Water Bill if applicable
Deductions
• Childcare (1 month of receipts or Statement from Day Care Provider; if personal provider and not a Day Care the letter must be notarized.)
• If you are over the age of 62 or Disabled you may bring in Pharmacy Statement of RX Medicines you purchased for 1 year; any out of pocket copays for doctors’ visits, procedures, hospital, etc.
Vehicle Information
Insurance, Tag Receipt, and Driver’s License
Pet Information
Vaccination record from the Veterinarian
Student Verification
Full-Time or Part-Time, you must bring in a current academic schedule of classes that you are enrolled in. If you receive a refund check after tuition and other fees are paid, you will also need to bring in proof of this. If you are 18 years old or older and in High School, we will need a letter from the office stating you are still a High School student and your anticipated graduation date.
5 years of residence history, including landlord, name and phone number.
